In this assessment, you will identify a patient/family/population health problem and determine how it affects locating credible databases and research quality of care, patient safety, and costs to the system and individual. You will also design an intervention to address the problem. In a 5-7 page written assessment, assess how health care technology, coordination of care, and community resources can be applied to address the patient/family/population health problem you’ve defined. You will also spend approximately 2 direct practicum hours exploring these aspects of the problem with the patient, family, or group you’ve chosen to work with and, if desired, consulting with subject matter and industry experts.

Identifying, designing, and implementing a health care solution that addresses the needs of a complex patient population requires more than just medicine and science. Having a clear understanding of the problem you are trying to solve can help you devise an appropriate and cost-effective plan of attack. The take my online course provides you with an opportunity to test your problem-solving skills in a structured environment. In a 5-7 page written assessment, you will need to demonstrate your mettle by identifying and presenting the most important aspects of your plan of attack as well as the most effective and efficient ways to implement the solution. Your results will be used to develop your project plans in a manner that will ensure that you meet or exceed the objectives for each component.

After defining your problem and planning your capstone project, you’ll explore the effect of your chosen patient, family, or population health problem on quality of care, patient safety, and costs to the system and individual. You’ll do this by focusing on leadership, collaboration, communication, change management, and policy.

 

Identify and explain best-practice strategies from the literature for communicating and collaborating with the patient, family, or group to improve their outcomes associated with your defined problem. In addition, describe how state board nursing practice standards and/or organizational or governmental policies influenced your work. In a separate written deliverable, develop an intervention to address your chosen patient, family, or population health issue as a solution to the problem. Submit this proposed intervention, with a 5-7 page analysis, to your faculty for review and approval. Throughout the course of your RN-to-BSN program, you’ll develop leadership, collaboration, communication, and change management skills that will help you to improve patient, system, and population outcomes. During this time, you’ll build the foundation for your professional product. As part of your capstone project, you’ll present your approved intervention to the patient, family, or group and record a video reflection about your experience in the practicum, the development of your  improvement plan in service presentation capstone project, and your personal and professional growth during the course of your RN-to-BSN experience. This video should be submitted in CORE ELMS, along with the patient or family/group feedback you’ve received about your intervention.

Assessing the effect of an intervention can be a challenge, but it’s possible to do it in a way that’s not just another research paper. Using a variety of tools, you’ll assess how well your new approach is doing as you track its success over time. For example, you might track its effectiveness through patient feedback or a survey of the health care community. Alternatively, you might monitor the impact of your intervention via metrics such as blood pressure readings or medication management efficiency. This assessment will take you about an hour to complete, but it’s worth the effort because it can be a useful tool for your future professional life. As a result, you’ll have a better chance at landing that coveted internship or entry-level position.
